Authorization to Release Student Information

Although you are registered to take courses at DeKalb High School of Technology-South, your records are held at your primary school. The Authorization to Release Student Information sheet is for your parent/guardian to complete. Your parent must put the name and address of your primary high school in the top section of the form. Then complete the rest of the form as indicated. 

Download and save the document. The form is a typeable pdf file and should be completed on your computer. Once the document is complete, upload and submit it.
